A pioneering approach to diabetes care.
The status quo of type 2 treatment isn’t working for people. Level2 is different.
Level2 is personalized and data-driven care for individuals with a type 2 diabetes diagnosis in the last 24 months, or are prescribed certain glucose-lowering medications (see Eligibility FAQ: “Who can join Level2?”). Through an individualized approach, guided by wearable technologies and personal coaching, Level2 focuses on the reduction of A1C, increasing time in range of glucose levels, and the possibility of type 2 diabetes remission.
Type 2 diabetes can’t be “cured” and there is a lot of confusion out there about what diabetes remission really means. If a person is in remission, it means they are no longer considered to have diabetes according to their hemoglobin A1C (measurement of glucose levels over the past 3 months). If that is true, a person may no longer have to take any medicines for their diabetes. Remission takes effort and persistence to achieve, but is attainable for many people.
Level2 is provided at no additional cost to eligible members as part of their covered health insurance plan.
The continuous glucose monitoring (CGM) system has a small sensor (about the size of a thread) that sits under the skin and measures glucose (sugar) levels. It sends glucose data wirelessly using the Dexcom Glucose Program app to a compatible smart device (see https://www.dexcom.com/faq/what-devices-and-software-are-compatible-dexcom-cgm-apps for a list of devices compatible with the Dexcom Glucose Program app), so a person can track glucose highs and lows at regular intervals and make informed decisions about diabetes management.
Mind. Body. Lifestyle. That is our whole-person health approach. Participants and their coaches (e.g. registered nurses, registered dietitians, licensed social workers) can discuss how everyday decisions make a big difference. Participants can fine tune the interactions they have with their coach through phone or messaging capabilities to create a support system that aligns with their lifestyles, schedules, and goals.
